I, the undersigned, authorize Winners Circle Collision and its employees to repair my vehicle and to operate it for testing or inspection purposes.

I understand that an express mechanic’s lien is acknowledged on the vehicle to secure payment for repairs.

I acknowledge that Winners Circle Collision is not responsible for any personal items left in the vehicle, including but not limited to electronics, cash, tools, sunglasses, or other valuables. I agree to remove all personal belongings before dropping off the vehicle.

I understand that rental vehicle expenses are not covered by Winners Circle Collision.

I acknowledge that a daily storage fee will apply after 72 hours from the time repairs are completed, if the vehicle is not picked up.

I understand that full payment is required before the vehicle can be released. I am solely responsible for securing payment from the insurance company or any other third party.

I authorize the insurance company to issue payment directly to Winners Circle Collision for all repairs, including any supplements.

I authorize Winners Circle Collision to endorse my name on any insurance checks issued for these repairs.
